Abstract

A 14-year-old girl presented with persistent pain in the upper arm for 4 days after a minor trauma. Radiographic examination of the right humerus revealed a centrally located cystic lesion surrounded by a thin cortex in the proximal diaphysis with a fallen fragment sign, in keeping with a simple bone cyst (SBC) with pathological fractures. A SBC is a benign self-limiting cystic bone lesion mainly affecting children and adolescents. It may cause pain and bone deformity due to repetitive pathological fractures. Management intends to prevent future fractures and secondary bone deformity. After 6 weeks of a conservative treatment, radiographic examination revealed a nearly complete sclerotic healing of the lesion.
