Medication review: Cystic fibrosis

Abstract

Sarah is 17 years old and was diagnosed with cystic fibrosis (CF) at birth. She takes multiple medicines to manage her CF, however adherence to prescribed treatments has become an issue due to her busy lifestyle. Today, Sarah complains of increased cough and sputum over the past 3 days. She has been diagnosed with a pulmonary exacerbation by her CF care team, only 3 months after a course of intravenous (IV) antibiotics. The CF care team is concerned about Sarah's adherence and knowledge of her current treatments. There is no dedicated CF pharmacist at the care centre, therefore referral has been made for a Home Medicines Review.
