Abstract

Load-compromised individuals constitute a diverse clinical population unified by health challenges limiting their participation in weight-bearing activities. This may include musculoskeletal injuries, arthritis, osteoporosis, neurological disorders, chronic pain, post-surgery recovery, and obesity. The incorporation of hypoxic conditioning into load-compromised individuals' rehabilitation signifies a paradigm shift, conferring health and performance benefits with minimal or no contractile activity. Hypoxic conditioning uniquely enhances cardiovascular or musculoskeletal exercise intensity without proportionally increasing the mechanical load on the locomotor system. This intervention holds the promise of mitigating cardiovascular deconditioning, muscle atrophy, and strength deficits, offering load-compromised individuals a pathway to conventional physical activity, minimizing reinjury risks, and facilitating a return to high-load exercises. As hypoxic conditioning modalities present a multifaceted landscape, understanding their impact on internal and external loads, their relevance across rehabilitation stages, and their influence on physiological responses in load-compromised individuals becomes crucial to enable precise, evidence-based formulations optimizing patient outcomes. This chapter delivers a proof-of-concept overview, detailing the clinical evidence and efficacy of both systemic and localized hypoxic conditioning treatments, whether applied passively or in conjunction with aerobic or resistance exercises across various load-compromised cohorts.
