We report the case of a 13-months-old girl, admitted because of fever and otorrhea. Clinical examination prompted a diagnosis of acute otitis media/mastoidis. A mastoidectomy was performed followed by intravenous administration of antibiotics. Postoperatively, she became lethargic and had spiking fever. An MRI scan revealed the diagnosis of a sigmoid sinus thrombosis. The clinical picture and treatment strategies are discussed based upon a review of the current literature.
